CONNECT (Continuous Engagement between Community and Clinic Treatment) is a health and wellness program that serves our most vulnerable adult communities with mental health care, help finding housing, and employment support in a flexible model that meets clients where they are.

• Substance recovery support
• Help with employment, education, housing application assistance, legal, and other needs
• Criminal-justice involved individuals are welcome
• Special events, workshops, information sessions, meet-and-greet social events, and food pantry (food only available for participants)
